Documenting Surgery

Surgery is the portion of an exam that summarizes treatments that require either manual or instrumental operations. This tutorial will demonstrate how to document surgery as part of a patient exam in Vision(X).

Surgery Menu


Begin the exam, then select the Surgery category to open the documentation fields. If this portion of the exam has already been documented by another provider, or will simply not be performed as part of today's exam, select the check boxes as appropriate.


CPT & SNOMED Codes

Confirm the date and use the toggles to indicate whether this was a surgery conducted on one or both eyes, then use the CPT(s) and SNOMED(s) fields to search for applicable codes as necessary:


Surgeon(s)


Use these fields to document the surgeon who performed the procedure(s). Search for a provider in your Vision(X) site, or add a new surgeon and indicate whether they are an internal or external provider:


Diagnosis


Use these fields to search for Pre-operation and post-operation diagnoses as necessary:


Anesthesia


If anesthesia was used, click the "Anesthesia" field to select any available option, then add details in the required "Anesthesia Meds" field:
