Begin the exam, then select the Binocular Assessment category to open the documentation fields. If any visual field testing was documented by a non-provider (technician) or will simply not be completed as part of this exam, select the check box for "To Review" or "Not Performed" as appropriate:
For each eye, select either "Full" or Abnormal" toggles to document conditions of the muscles attached to the outside of the eye. All fields can be marked as "Normal", and they can quickly re-set using the "Clear All" button:
Near point of convergence measures the closest distance at which the eyes can focus on a single object together before vision blurs and/or breaks, as well as the distance at which clear vision is recovered. Enter values for each field manually, or use the slider option provided. All fields can be quickly re-set using the "Clear All" button:
Stereo
Stereo documents the depth of perception created when the brain combines two slightly different images produced by two eyes into a single, three-dimensional picture. Beginning with the Randot test, enter values as applicable. All fields can be marked as normal or cleared as needed using the buttons provided. Additional testing for Stereo fly, Reindeer, and Randot testing are available using the "+Add new" button.
Cover Test
Cover test document the degree of exphoria at both distance and near fixation of the eyes. Select Primary or Secondary gaze toggle and either "CC" (with correction) or "SC" (without correction), then enter values as applicable. Values can be added manually, or by using the slider provided. All fields can be marked as normal or cleared as needed using the buttons provided.
